You are welcome to download and fill out the required forms before your appointment and bring them with you.
What to Bring to Your Appointment
To ensure a seamless check-in process, please bring the following items with you:
- Your medical insurance card(s)
- A photo ID
- A list of your current prescriptions
- Any previous hearing test records that you may have for review
