To expedite the check-in process for your upcoming appointment at the Hearing Center at Eye Consultants of Atlanta, please complete and bring in the appropriate forms below. Please bring the items below to your upcoming visit:
- Your medical insurance card(s)
- A photo ID
- A list of your current prescriptions
- Any previous hearing test records that you may have for review
- Patient Intake Form
